Lamoille County Shared Living Provider Opportunity

Job description: 

GMSS is looking for someone with compassion and dedication to join our team as a Shared Living Provider (SLP) for an individual with Developmental Disabilities.  Ideally, this SLP would move into the participant’s apartment in Lamoille County, Vermont. Considerations may be made for homes that are or could become fully wheelchair accessible. 

This individual requires support with: 

• Personal Care 
• Independent Activities of Daily Living 
• Coordination, Transportation and Advocacy to maintain Health and Wellness 
• Medication Administration 
• Community Inclusion 

The Shared Living Provider must be able to work collaboratively within a team of existing supports and possess the ability to provide a safe, comfortable home environment that fosters autonomy and a Person-Centered approach to care. 

Compensation consists of a tax-free Difficulty of Care stipend and a monthly taxable Room and Board payment. Respite and Community Support funds are available to support this individual through a Vermont DS Waiver. 

$55,000+ Annually
